Diversity, Equity, and Inclusion Survey

Hello,

We would like to ask your participation in completing a survey on Diversity in
Shared Research Core Facilities.  This survey will help us to understand more
about diversity, equity and inclusion within core facilities. If you could
please share this survey with the core facilities within your Institution we
would greatly appreciate your support.

Relevance: Diversity within scientific careers has been given minimal
attention, yet there are discussions prevalent throughout our organizations in
which we work to train and education individuals about diversity.  We are
aiming to learn more about organizational programs, personal lived experiences,
and the understanding of diversity from the core facility role.

Purpose: We are seeking at least 300 participants in this survey of individuals
that work within or help to oversee a core or shared facility.  These roles may
include an individual that classifies themselves as a Core or Shared Facility
Director, Manager, or Staff member of a core or shared facility, as well as
roles that help to provide administration over core or shared facilities within
their organization.

Data: Data collected will be used to gain a better sense of the impact of
diversity training, awareness, and experiences on individuals that work within
the core or shared facility environment.  The de-identified results of the
survey will be disseminated broadly.

Risk/Benefit:  There is little to no risk associated with the completion of
this study by the participant.  You are able to discontinue your participation
in this survey by closing it at any time.  Participants may also contact us
with a request to withdraw their survey responses at any time.  You will not be
penalized for your decision to participate or not.  This survey will be used to
help inform researchers on diversity from the perspective of individuals
working within the core facility environments.

Instruction: Individuals that work within or provide oversight to a core or
shared facility are asked to answer all questions accurately to the best of
their knowledge and how it relates to them.  Please do not guess, if you feel
you need to, just leave the question blank if you don't know the answer.  This
survey should not take more than 15 minutes to answer
